This form is for use by USGenWeb State or Special Project Coordinators ONLY.
The form may be used for quarterly updates, or any time there are changes to your Project Membership List.
The EC encourages all Project Coordinators to keep their membership lists current by reporting changes when and as they occur.

Individual members please use the Registration Form to update your Member Registration.

Please enter information in the following manner:
Last name, Given Name, E-mail address, alternate E-mail address(es), State (initials) or Special Project area, Position**, County, or County/Town, Date when change occurred.

**State Positions
SC - State Coordinator
ASC - Assistant State Coordinator
CC, Co-CC, ACC - County Coordinator
TC - Town Coordinator
LC - Local Special Project (Include name of state special project)

**Special Project Positions
SFM - State File Manager, Archives
FM - County File Manager, Archives
PC - Head of Natl Special Project
LPC - Local Coord for Natl Special Project

If there are no changes for a quarterly update, simply enter "no changes" in the appropriate box(es) below.
